Özet

Objective: Regenerative endodontic treatments are applied to permanent teeth with incomplete

root development, typically when the root formation has completed approximately two-thirds of its

development. This case report aims to present the regenerative endodontic treatment of an

immature permanent tooth with apical periodontitis.

Case Report: A 12-year-old patient presented to our clinic with complaints of spontaneous

nocturnal pain in tooth #35. Clinical examination revealed percussion sensitivity, and periapical

radiographic evaluation confirmed apical periodontitis. Regenerative endodontic treatment was

considered for the patient. In the first session, the infected pulp tissue was extirpated under aseptic

conditions, and the root canal was irrigated with 20 mL of 2% sodium hypochlorite and 0.9% saline

solution, following the first protocol of regenerative endodontic treatment. In the second session,

two weeks later, the root canal was irrigated with 20 mL of 17% EDTA solution and 0.9% saline

solution. Apical bleeding was then induced, and coronal sealing was achieved with a 3-mm layer of

MTA. Permanent restoration was completed using composite material. However, during a follow-up

session, a fracture was observed in the initial restoration, and the restoration was replaced with a

stainless steel crown. At the 6-month and 1-year follow-ups, continued root development was

observed, and the symptoms showed both clinical and radiographic resolution.

Conclusion: The reported case demonstrates that regenerative endodontic treatment procedures

can be successfully applied to immature necrotic permanent teeth. Further studies on this treatment

approach are recommended in the literature.
